Thank you for your interest in submitting a Healthcare Access Issue Report. Bleeding Disorders Council of California (BDCC) created this Report in order to learn more about barriers individuals have experienced in obtaining access to the appropriate care/treatment of their bleeding disorder. To help us better understand these barriers, we request you complete the entire report and provide as much pertinent information as possible.

Although some information, such as your name and contact information is optional, please know that this information may be useful to us and should you include this information on the form, your identity will not be disclosed to any other individuals, groups or organizations outside of BDCC without your express written permission.

BDCC will be using the data collected from the reports to look for trends and to help us identify recurring healthcare access issues. BDCC will then target these issues and work on devising possible solutions. In some cases, such as access issues involving Medi-Cal, California Children’s Services (CCS) or Genetically Handicapped Persons Program (GHPP), BDCC may be able to help resolve your issue. And while we cannot resolve all access issues, we will work with individuals and entities to find solutions whenever possible.
